the Stoker Area Fall Retreat. We are delighted that you will be joining
us. We are planning an inspirational and motivational weekend, and we want
to thank you for helping to make that happen. By giving us as much of the
information below as you can, we can ensure that we meet your needs.
Registration
Information:
The
cost to attend the Fall Retreat is $189.00 (taxes included). Husbands’
registrations are $159.00.
Payment
can be made by VISA, MC, Amex or personal cheque. Make cheques payable
to “Angie Stoker.”
Please
completely fill out the attached registration. Incomplete registrations
can lead to incorrect preparations being made on your behalf, and may be
returned.
Space
available, you will be allowed to register on-site. Please don’t rely on
this however. We will sell out!
Refund
Policy: **
A full
refund will be offered if your cancellation is received by September 15th.
A $25.00
service charge (+ $3.25 HST) will be deducted from all refunds for cancellations
received from September 16th to October 5th.
No
refunds are possible after October 5th. An effort will be made to sell
your registration to another participant, with a $25.00 re-registration
penalty.

Please understand that we enter into financial contracts with Hotel du
Lac that bind us to providing numbers of people that we will be having
at the Retreat. The hotel then makes arrangements based on those obligations.
If fewer people come, we still have to pay for all of the arrangements
made on our behalf. Therefore, the earlier we receive a cancellation, the
more flexibility we have to refund the money and replace the registration
or adjust our commitment to the hotel.

It is important to ensure we have enough translation equipment to cover
everyone. All of our teachers and speakers will speak in their mother tongue,
which means if you are not bi-lingual, you will probably need translation

Hotel Accommodations:
To receive the preferred rates we have negotiated, hotel accommodations
must be reserved through us. We will use the accommodations of the Hotel
du Lac and will "overflow" into a sister hotel called Cap Tremblant Mountain
Resort. Each hotel has one and two bedroom condo suites. The one bedroom
suite includes a separate (private) bedroom, bathroom, a small kitchen
(with fridge, stove, microwave, cofee maker, dishes etc.), dining area,
living room with fireplace and a balcony overlooking Lac Tremblant, with
the ski hill and resort development as a backdrop. The living room
has either a convertible sofa or a Murphy bed for two more, making the
suite a suitable accommodation for 4 people. The two bedroom suites
have an additional private bedroom with an en suite bathroom, making it
suitable for six people.
The cost of the accommodation
is not included in the registration and you are not required to stay in
the hotel. If you have a house or condo or friends in the area, you can
stay there. Just click the "Not staying at the hotel" button and
ignore the remainder of this section.

The Card Security Code,
which is also known as a Credit Card ID (CCID) is often asked for by merchants
for them to secure "card not present" transactions occurring over the Internet,
by mail, fax or over the phone. Using the new authorization process I have
just switched to, and due to increased attempts at card fraud worldwide,
it is now mandatory to provide this code when the cardholder is not present.
Please enter the CSC in the
grid at left. If you have a three digit CSC, mark the furthest-right column
as empty.
The CSC is located on
the back of MasterCard and Visa. It is typically a separate group of 3
digits to the right of the signature strip.
On American Express
cards,
the CSC is a printed (NOT embossed) group of
4 digits on the front towards the right.

NOTE:You
are probably wondering why I chose to ask for credit card numbers the way
I did in the grid above. It isn't usually considered safe for you to give
credit card numbers over email, but the way I have done it converts the
numbers you give me into meaningless characters, and transmits them to
me as if they were answers to a multiple choice survey. Only when I enter
them into my records do they become numbers again. That way, your credit
card number is safe, even if it is intercepted. Secure sites are a magnet
for hackers. Every site which hackers have broken into to steal credit
card numbers was "secure".

The grid is filled out from
left to right, using each column of dots to record the digits of your credit
card number. Imagine your number written above the grid, as depicted below,
for the Visa number 4500 8565 4374 3685. Note that only 3,4 and 5 are available
in the first column since I only accept Visa(which all start with 4), MasterCard
(which start with 5) and Amex (which start with 3). When I receive the
message, it will look like you filled out a survey.
